The Practice offers a number of services online. If you wish to, you can:

• Book appointments on line
• Request repeat prescriptions
• View elements of your medical record

The elements of your medical record that are currently available for you to view are:

• Medications – current and past
• Allergies
• Immunisation
• Test results

In addition, you may also apply to view:

• Consultations (detailed coded information only)

Evidence shows that patients who are informed and involved in their own care have better health outcomes and are less likely to be admitted to hospital. By having access to your medical record online you will be empowered to take greater control of your health and wellbeing.

How to Register for Patient Access

To use the online services you will first need to register. You will be required to complete an application form. Please note to apply for this service you must be over 16 years old.

Print out an application form (link below) and return the form to the surgery (either by post, post through the letter box or email to us misbourne.practice@nhs.net or stgiles.surgery@nhs.net . You will then be contacted by the surgery.

If you have already registered for online access for booking appointments and/or requesting repeat prescriptions you will now be able to see medications, allergies, immunisations and test results. YOU WILL NEED TO MAKE A SEPARATE REGISTRATION REQUEST TO VIEW THE DETAILED CODED INFORMATION IN YOUR CONSULTATIONS.
